The Charter Standard Annual Health Check Form can be generated in 3 ways.
- Post and Email - Please contact Matt Handley on 0114 241 4992 or at matthew.handley@sheffieldfa.com to request a copy of your health check form. If your preferred method is post, then you will need to provide an address. If your preferred method is email, then an email address will have to be provided.
- Online - through accessing members services. This is by clicking on the 'members' tab in the top right hand corner of this article. For this you will need your FAN (FA Number). All registered Chairman, Secretaries, Treasurers and Welfare Officers have an FAN. If you do not know your FAN, please contact any member of staff at the County FA.
Regardless of the method chosen, all documentation will be in PDF, this means that all amendments will have to be made by hand. Amendments cannot be made electronically on PDF files.

As of the 2011/12 season you can access or request your Annual Renewal form on the dates below depending on which league your club is active within. In the case of many clubs who take part in a variety of different leagues you can chose which date is suitable for you.
- 1st September - Sheffield and Hallamshire Girls County League and Millennium League.
- 1st November - Sheffield and District Fair Play League, Sheffield Women’s League, Adult Teams and all clubs affiliated to Sheffield FA but play in leagues outside our boundaries.
- 1st February - Doncaster and District Junior League, Sheffield and District Junior League and Under 18’s League.
- 31st March - Deadline for all applications
Sheffield FA run clinics to support their Charter Standard Clubs through this process and also support clubs to gain the status in the first instance. Future clinics will take place at the Sheffield FA offices on:
- 19th January 2011
- 16th March 2011
- 18th May 2011
All clinics run from 5pm – 7pm and you are required to book a 15 minute slot, to do this please contact Matt Handley on 0114 241 4992 or at matthew.handley@sheffieldfa.com.
